Affiliate Terms & Conditions

1. Program Overview

The OurHealth Rx Affiliate Program (“Program”) allows approved participants (“Affiliates”) to earn commissions by referring customers to OurHealth Rx telehealth services. By participating in the Program, you agree to these Terms and Conditions.

2. Eligibility

To participate, you must:

• Be at least 18 years of age
• Have a valid PayPal account or U.S. bank account for payouts
• Have a valid Social Security Number or EIN for tax reporting (W-9 required)
• Comply with all applicable federal, state, and local laws
• Not be a current employee or contractor of OurHealth Rx

3. Commission Structure

Commissions are calculated on the net revenue of referred customer's first-month subscription. Recurring commissions (10%) apply for GLP-1 and Menopause services for the duration of the customer's active subscription, up to 12 months.

4. Tracking and Attribution

Each affiliate receives a unique tracking link with a 60-day cookie window. A conversion is attributed to the affiliate whose link was most recently clicked before the customer completed their purchase or enrollment.

Self-referrals (referring yourself or family members in the same household) are prohibited and will result in commission forfeiture and possible account termination.

5. Payment Terms

• Payouts are processed on the 15th of each month for the previous month's verified commissions
• Minimum payout threshold: $50
• Payment methods: PayPal or ACH direct deposit
• A completed W-9 form is required before first payout
• Commissions on refunded orders will be reversed
• OurHealth Rx reserves the right to hold commissions for up to 30 days for fraud review

6. FTC Disclosure Requirements

All affiliates must clearly disclose their affiliate relationship with OurHealth Rx in any content promoting our services. This includes but is not limited to:

• Social media posts (Instagram, TikTok, Facebook, YouTube)
• Blog posts and website content
• Email newsletters
• Video content

Acceptable disclosure language: “#ad” “#sponsored” “Affiliate link — I earn a commission if you sign up” or similar clear language.

7. Prohibited Activities

The following activities will result in immediate termination and forfeiture of commissions:

• Making false or misleading claims about OurHealth Rx services
• Bidding on OurHealth Rx branded keywords in paid advertising without written approval
• Spam email campaigns or unsolicited text messages
• Cookie stuffing or other fraudulent tracking manipulation
• Promoting services to minors
• Making medical claims beyond what is approved in our content guidelines
• Self-referrals or referring household members

8. Program Modifications and Termination

OurHealth Rx reserves the right to modify commission rates, program terms, or terminate the program at any time with 30 days written notice. We reserve the right to terminate individual affiliate accounts immediately for violations of these terms.

9. Governing Law

These Terms are governed by the laws of the State of Tennessee. Any disputes shall be resolved through binding arbitration in Shelby County, Tennessee.
